Fringe benefit reporting forms are now available in excel format, which will perform the calculations for you. Please complete the information on the top of the form, including Company name, address, phone number, job location, etc. Next, enter the information in the shaded area of the table, adding more lines as necessary. The totals will be automatically calculated for you. If you need any help in completing the form, you may contact the fund office at (315) 492-1796 or contact your district office.
